The following brief questionnaire is used around the world to help doctors and health-care providers determine if their patients are at risk for drinking related problems.

When the question asks about your drinking, keep in mind the following guideline: a standard drink is equal to

    • 12 oz. (355 ml.) of beer (5% alcohol)
    • 5 oz. (150ml.) of wine (12% alcohol)
    • 1.50 oz. (45ml.) of liquor (40% alcohol or 80 proof)

It's important to remember that because, for example, a pint of beer is actually 1¼ drinks, a cocktail frequently contains at least two shots, and a bottle of wine actually has 5 standard drinks. To get accurate feedback, you won't want to underestimate your drinking.
